Supported Independent Living Program

We support 30+ people to live independently in the community, working closely with related organizations to make sure each person has the support they need.

Accommodation

  • Finding and maintaining suitable accommodation.
  • Building independent living skills.

Financial

  • Applying to ODSP, Ontario Works and other income sources.
  • Banking, budgeting and money management.
  • Maintaining sufficient funds for housing, basic needs and recreation.

Health care

  • Finding regular health care of all types.
  • Transportation and support during appointments when needed.
  • Supporting choices that promote a healthy lifestyle.

Social and employment

  • Finding social activities and personal interests.
  • Maintaining connections with family and friends.
  • Connecting with services to find and maintain employment.

Case management

  • Continuity and coordination of service.
  • Ongoing follow-up and regular review of individual goals.
  • Referrals and access to other community services.
